The Support Coordination Team at CLA provides both Level 2 Coordination of Supports and Level 3 Specialist Support Coordination to people with a cognitive disability and/or psychosocial disability.
Our Support Coordinators and Specialist Support Coordinators provide holistic, proactive, adaptive, inclusive and reflective support to constituents that reside in Brisbane, Moreton Bay and Sunshine Coast Regions.
All members of our Support Coordination Team holds a Tertiary Degree in Allied Health (Social Work, Psychology and Public Health). This allows us to provide holistic and professional support to constituents utilising CLA Practice Principles, CLA Capacity Building Support Framework, Support Coordination Practice Framework, Relationship Based Practice and Trauma Informed Practice. Through our co-ordinated approach, we aim to improve the lives of our constituents by facilitating the most beneficial support services.
